Provider Requirements
PhlebFlow is a free directory listing for independent mobile phlebotomists and CLIA laboratories. Here is exactly what you need to join, and how our review and verification process works. Signing up takes about 5 minutes.
Who can join
- Independent mobile phlebotomists — solo practitioners or small mobile teams.
- Laboratories — mobile or CLIA-waived/certified labs offering collection services.
What you need to sign up
- Your contact information (name, phone, email).
- Your service area — the cities you cover (we expand them to ZIP-level coverage for patient matching).
- The services you offer (at-home draws, specialty kits, pediatric, specimen pickup, and more).
- Your certification, from our accepted list: ASCP, NHA, AMT, NCCT, AMCA, RPT, a state certification, another registry or private certification, or a training certificate.
- A short bio for your public profile.
- An accuracy attestation: you confirm your profile information is accurate and that you maintain the credentials you list.
State license rules
A state phlebotomy license is legally required in only four states. If you practice in one of them, you must provide your license ID to list:
- California — CPT I/II (CDPH)
- Washington — Medical Assistant–Phlebotomist (WA DOH)
- Nevada — Laboratory Assistant license
- Louisiana — LSBME phlebotomist license
Everywhere else (including Florida), national certification is voluntary but strongly recommended — it is what patients look for on your profile.
Laboratories: CLIA required
Laboratory listings require a CLIA number. We verify it against the CMS registry (qcor.cms.gov) before your listing is published — a lab listing without a validated CLIA number will not go live.
How verification works
- Every new listing is reviewed before publishing. Until it is approved, your profile is hidden from patient searches.
- Credentials on profiles are self-reported by the provider — except where a profile shows the "Credential verified" badge, which PhlebFlow grants only after manually checking that credential.
- PhlebFlow is a directory and logistics platform. We do not employ providers and do not guarantee licensure, availability, or pricing — patients are always advised to verify credentials directly with their chosen provider.
Professional liability insurance
Insurance is not required to join — but carrying professional liability insurance is strongly recommended, and insured profiles will qualify for trust badges in the future.
