These terms are for the professionals and businesses listed on PhlebFlow. Patients and other visitors are covered by the Terms of Service and the Privacy Policy, which also apply to you.

1. Who these terms are for

These Provider Terms apply to every phlebotomist, mobile phlebotomy company and laboratory that lists a profile on PhlebFlow (the "platform"). They sit alongside the Terms of Service and Privacy Policy that apply to everyone who uses the site, and they govern the listing itself: what we publish about you, what we send you, and what each side may and may not do.

PhlebFlow is a directory and logistics platform. It does not perform, supervise, schedule on your behalf, or bill for any blood draw. You are an independent professional or business — not an employee, agent, partner, or contractor of PhlebFlow — and nothing in these terms or in the platform's design changes that.

2. Your listing

Listing is free and carries no commission, referral fee, or percentage of anything you charge. That does not change with any paid plan (section 7).

By listing, you allow PhlebFlow to display the information you submit — your name or business name, photo or logo, city and coverage ZIP codes, services, fees, hours, languages, credentials and bio — on your profile page, on the pages that list providers by city, state, service or kit, in search results, and in the emails the platform sends about your listing. You keep every right in that content; we use it only to present your listing and can stop using it the moment you remove it.

You are responsible for what your listing says. Fees, hours and coverage are yours to set and to keep current; a patient who books against a stale fee will hold you, not us, to the number they saw. You may edit your listing from your dashboard at any time, and you may ask us to remove it at any time.

3. Credentials and verification

You confirm, at signup and whenever you edit your profile, that you hold the certification or license you list and that it is current. Where a state licenses phlebotomists, you confirm you hold that state's license for every state you cover, and you agree to remove coverage in any state where you do not.

PhlebFlow may check a credential number against the registry that issued it. A profile shows the "Credential verified" badge only after that check succeeds; otherwise the credential is shown as self-reported. When a registry publishes an expiration date, the badge is withdrawn automatically on that date until the renewal is on record. We may ask you for a renewal, a certificate of insurance, or a correction, and we may unpublish a listing whose credential cannot be confirmed or has lapsed.

We do not verify liability insurance, background checks, or clinical competence, and your listing never claims that we do. Anything a patient or an organization needs beyond the credential check, they will ask you for directly.

4. Patient requests and your conduct

A patient request routed to you is an invitation, not an obligation. You may accept or decline any request. If you neither accept nor decline within the response window shown in your dashboard, the request may be forwarded to another listed provider and the patient told so.

Once you accept, the patient's contact details are shown to you for that request only. You may use them to arrange, perform, and follow up on that visit, and for nothing else: no marketing, no lists, no sharing with anyone the visit does not require. Delete them from your own records when your own retention obligations allow.

You are the healthcare provider in that visit. Obtaining the patient's consent to the draw, verifying the lab order, handling and transporting specimens, HIPAA and state privacy obligations for what you learn during the visit, scope-of-practice limits, and every clinical judgment are yours. The platform holds no lab order, no result, and no diagnosis, and must not be sent any.

Treat patients as you would want a member of your family treated. A report of abusive, discriminatory, unsafe or deceptive conduct is investigated, and can end a listing.

5. Pricing and payment

You set your own fees and collect payment directly from the patient or organization, by the method you agree with them. PhlebFlow is not a party to that payment, holds no funds, and issues no refunds. Any cancellation, late, or no-show fee is yours to set and yours to disclose before the visit; a fee a patient was never told about is a dispute you will have with the patient, not with us.

Do not quote, on or off the platform, a fee that differs from your listing without telling the patient before the visit.

6. Reviews

Reviews come from two sources: a private link the platform sends a patient after a visit booked here (shown as "Verified patient"), and a link you create in your dashboard for a patient you have already served (shown as "Invited by the provider"). Every review is moderated before it appears. You may only invite people who actually had a visit with you; you may not offer anything in exchange for a review, write one yourself, or ask a patient to change or remove one. Doing so ends your ability to invite reviews and can end the listing.

7. Paid plans

PhlebFlow may offer paid plans (for example, a featured placement or a team listing). A plan fee pays for software and placement — never for a referral, a lead, or a share of what you charge — and a paid plan never changes which credentials are shown as verified. Plans are month to month unless stated otherwise, can be cancelled from your dashboard at any time to take effect at the end of the paid period, and their prices can change only with notice before your next renewal. A plan does not create an employment or agency relationship, and does not guarantee any number of patients.

8. Suspension and removal

We may pause or remove a listing when a credential cannot be confirmed or has lapsed; when a listing covers an area far beyond where you actually travel; when a report about a visit is substantiated; when the listing has been inactive or unresponsive for an extended period; or when these terms are otherwise broken. Where possible we tell you what is missing first and give you the chance to fix it. You may remove your own listing at any time from your dashboard or by writing to [email protected].

Removal does not erase the record of requests you accepted or reviews patients left; those are kept as long as the Privacy Policy states.

9. Responsibility

PhlebFlow is not liable for the outcome of any visit, for a patient who cancels or does not pay, for a request that turns out to be false, or for any loss arising from your services. You are responsible for your own licenses, insurance, taxes, equipment, and for any claim that arises from a visit you performed, and you agree to hold PhlebFlow harmless from such claims. Nothing here limits liability that cannot be limited under applicable law.

10. Changes and questions

When these terms change, the version date above changes with them and your dashboard asks you to accept the new version. Continuing to keep a listing after that request means you accept them. Questions go to [email protected]; a formal notice, to [email protected].
